Hospital discharge: death of patient with addiction

NHS Greater Glasgow and Clyde did not “adequately consider” the vulnerability of a patient with addiction issues who died soon after discharge from hospital. He had received addiction treatment but his parents complained to the Scottish Social Services Ombudsman about failure to communicate with addiction and social services before discharge.
